One of the changes necessary for shared page tables is to standardize the
pxx_page macros. pte_page and pmd_page have always returned the struct
page associated with their entry, while pte_page_kernel and pmd_page_kernel
have returned the kernel virtual address. pud_page and pgd_page, on the
other hand, return the kernel virtual address.
Shared page tables needs pud_page and pgd_page to return the actual page
structures. There are very few actual users of these functions, so it is
simple to standardize their usage.
Since this is basic cleanup, I am submitting these changes as a standalone
patch. Per Hugh Dickins' comments about it, I am also changing the
pxx_page_kernel macros to pxx_page_vaddr to clarify their meaning.

Back when pci_dev had base_address[], loop of form
base = &...->base_address[0];
for (.....) {
...

}
was fine, but when that array got spread in ->resource[...].start
replacing the initialization with
base = &...->resource[0].start;
was not a sufficient modification. IOW this code got broken for cases
when there had been more than one resource to fill. All way back in

This prevents cross-region mappings on IA64 and SPARC which could lead
to system crash. They were correctly trapped for normal mmap() calls,
but not for the kernel internal calls generated by executable loading.
This code just moves the architecture-specific cross-region checks into
an arch-specific "arch_mmap_check()" macro, and defines that for the
architectures that needed it (ia64, sparc and sparc64).
Architectures that don't have any special requirements can just ignore
the new cross-region check, since the mmap() code will just notice on
its own when the macro isn't defined.

There is an implicit assumption in the code that ranges will translate
to something that can fit in 2 32-bit cells, or a 64-bit value. For
certain kinds of things below PCI this isn't necessarily true.
Here is what the relevant OF device hierarchy looks like for one of
the serial controllers on an Ultra5:
Node 0xf005f1e0
ranges: 00000000.00000000.00000000.000001fe.01000000.00000000.01000000
01000000.00000000.00000000.000001fe.02000000.00000000.01000000
02000000.00000000.00000000.000001ff.00000000.00000001.00000000
03000000.00000000.00000000.000001ff.00000000.00000001.00000000
device_type: 'pci'
model: 'SUNW,sabre'
Node 0xf005f9d4
device_type: 'pci'
model: 'SUNW,simba'
Node 0xf0060d24
ranges: 00000010.00000000 82010810.00000000.f0000000 01000000
00000014.00000000 82010814.00000000.f1000000 00800000
name: 'ebus'
Node 0xf0062dac
reg: 00000014.003083f8.00000008 --> 0x1ff.f13083f8
device_type: 'serial'
name: 'su'
So the correct translation here is:
1) Match "su" register to second ranges entry of 'ebus', which translates
into a PCI triplet "82010814.00000000.f1000000" of size 00800000, which
gives us "82010814.00000000.f13083f8".
2) Pass-through "SUNW,simba" since it lacks ranges property
3) Match "82010814.00000000.f13083f8" to third ranges property of PCI
controller node 'SUNW,sabre', and we arrive at the final physical
MMIO address of "0x1fff13083f8".
Due to the 2-cell assumption, we couldn't translate to a PCI 3-cell
value, and we couldn't perform a pass-thru on it either.
It was easiest to just stop splitting the ranges application operation
between two methods, ->map and ->translate, and just let ->map do all
the work. That way it would work purely on 32-bit cell arrays instead
of having to "return" some value like a u64.
It's still not %100 correct because the out-of-range check is still
done using the 64 least significant bits of the range and address.
But it does work for all the cases I've thrown at it so far.

Remove the special implementation for nr_pagecache and make it a zoned counter

Happily, life is much simpler on 32-bit sparc systems.
The "intr" property, preferred over the "interrupts"
property is used-as. Some minor translations of this
value happen on sun4d systems.
The stage is now set to rewrite the sparc serial driver
probing to use the of_driver framework, and then to convert
all SBUS, EBUS, and ISA drivers in-kind so that we can nuke

This ugly hack was long overdue to die.
It was a way to print out Sparc interrupts in a more freindly format,
since IRQ numbers were arbitrary opaque 32-bit integers which vectored
into PIL levels. These 32-bit integers were not necessarily in the
0-->NR_IRQS range, but the PILs they vectored to were.
The idea now is that we will increase NR_IRQS a little bit and use a
virtual<-->real IRQ number mapping scheme similar to PowerPC.
That makes this IRQ printing hack irrelevant, and furthermore only a
handful of drivers actually used __irq_itoa() making it even less
useful.

While cleaning up parisc_ksyms.c earlier, I noticed that strpbrk wasn't
being exported from lib/string.c. Investigating further, I noticed a
changeset that removed its export and added it to _ksyms.c on a few more
architectures. The justification was that "other arches do it."
I think this is wrong, since no architecture currently defines
__HAVE_ARCH_STRPBRK, there's no reason for any of them to be exporting it
themselves. Therefore, consolidate the export to lib/string.c.

This fixes one build error introduced in sparc with the patch of Oct 30,
resent Nov 4 "[patch 3/5] atomic: atomic_inc_not_zero" I still can't get
sparc to build, but at least it gets further after I remove this line.
Apparently, this change was agreed to by Andrew and Nick on Nov 14, but
everyone thought someone else was doing it.

Earlier I unifdefed PageCompound, so that snd_pcm_mmap_control_nopage and
others can give out a 0-order component of a higher-order page, which won't
be mistakenly freed when zap_pte_range unmaps it. But many Bad page states
reported a PG_reserved was freed after all: I had missed that we need to
say __GFP_COMP to get compound page behaviour.
Some of these higher-order pages are allocated by snd_malloc_pages, some by
snd_malloc_dev_pages; or if SBUS, by sbus_alloc_consistent - but that has
no gfp arg, so add __GFP_COMP into its sparc32/64 implementations.
I'm still rather puzzled that DRM seems not to need a similar change.

Although we tend to associate VM_RESERVED with remap_pfn_range, quite a few
drivers set VM_RESERVED on areas which are then populated by nopage. The
PageReserved removal in 2.6.15-rc1 changed VM_RESERVED not to free pages in
zap_pte_range, without changing those drivers not to set it: so their pages
just leak away.
Let's not change miscellaneous drivers now: introduce VM_UNPAGED at the core,
to flag the special areas where the ptes may have no struct page, or if they
have then it's not to be touched. Replace most instances of VM_RESERVED in
core mm by VM_UNPAGED. Force it on in remap_pfn_range, and the sparc and
sparc64 io_remap_pfn_range.
Revert addition of VM_RESERVED to powerpc vdso, it's not needed there. Is it
needed anywhere? It still governs the mm->reserved_vm statistic, and special
vmas not to be merged, and areas not to be core dumped; but could probably be
eliminated later (the drivers are probably specifying it because in 2.4 it
kept swapout off the vma, but in 2.6 we work from the LRU, which these pages
don't get on).
Use the VM_SHM slot for VM_UNPAGED, and define VM_SHM to 0: it serves no
purpose whatsoever, and should be removed from drivers when we clean up.

Introduce an atomic_inc_not_zero operation. Make this a special case of
atomic_add_unless because lockless pagecache actually wants
atomic_inc_not_negativeone due to its offset refcount.

a) in smp_lock.h #include of sched.h and spinlock.h moved under #ifdef
CONFIG_LOCK_KERNEL.
b) interrupt.h now explicitly pulls sched.h (not via smp_lock.h from
hardirq.h as it used to)
c) in three more places we need changes to compensate for (a) - one place
in arch/sparc needs string.h now, hardirq.h needs forward declaration of
task_struct and preempt.h needs direct include of thread_info.h.
d) thread_info-related helpers in sched.h and thread_info.h put under
ifndef __HAVE_THREAD_FUNCTIONS. Obviously safe.

Make some changes to the NEED_RESCHED and POLLING_NRFLAG to reduce
confusion, and make their semantics rigid. Improves efficiency of
resched_task and some cpu_idle routines.
* In resched_task:
- TIF_NEED_RESCHED is only cleared with the task's runqueue lock held,
and as we hold it during resched_task, then there is no need for an
atomic test and set there. The only other time this should be set is
when the task's quantum expires, in the timer interrupt - this is
protected against because the rq lock is irq-safe.
- If TIF_NEED_RESCHED is set, then we don't need to do anything. It
won't get unset until the task get's schedule()d off.
- If we are running on the same CPU as the task we resched, then set
TIF_NEED_RESCHED and no further action is required.
- If we are running on another CPU, and TIF_POLLING_NRFLAG is *not* set
after TIF_NEED_RESCHED has been set, then we need to send an IPI.
Using these rules, we are able to remove the test and set operation in
resched_task, and make clear the previously vague semantics of
POLLING_NRFLAG.
* In idle routines:
- Enter cpu_idle with preempt disabled. When the need_resched() condition
becomes true, explicitly call schedule(). This makes things a bit clearer
(IMO), but haven't updated all architectures yet.
- Many do a test and clear of TIF_NEED_RESCHED for some reason. According
to the resched_task rules, this isn't needed (and actually breaks the
assumption that TIF_NEED_RESCHED is only cleared with the runqueue lock
held). So remove that. Generally one less locked memory op when switching
to the idle thread.
- Many idle routines clear TIF_POLLING_NRFLAG, and only set it in the inner
most polling idle loops. The above resched_task semantics allow it to be
set until before the last time need_resched() is checked before going into
a halt requiring interrupt wakeup.
Many idle routines simply never enter such a halt, and so POLLING_NRFLAG
can be always left set, completely eliminating resched IPIs when rescheduling
the idle task.
POLLING_NRFLAG width can be increased, to reduce the chance of resched IPIs.
